So, as far as I know, this particular Spider-Man only appeared in the comics in a single issue. That issue being Amazing Spider-Man vol 3 #9, which was Part 1 of Spider-Verse.
As you can see, he’s not in good shape, both in health and physical shape. At some point this Spider-Man was turned into a monstrous nosferatu style vampire, but even that was not enough to save him from the deadly and voracious Inheritors. Here we can see he has been served up on a golden platter at the Inheritor dinner table. The Inheritor Brix comments that this particular dish is quiet ironic, stating “My offering has the sweet taste of irony, father. My Spider-Totem is also a Vampire. But it is we who shall suck out his life force. Brilliant, right?”
It is widely believed that this version of Spider-Man is based on a spooky series of Spider-Man toys by Toy Biz. However, that action figure doesn’t look that much like the image in the comic. However, after little further research, I found a post from 2010 in a Custom Action Figure forum that I believe this Spider-Man is truly based on. Check out both toys, the original on the left, and the custom made one on the right. You be the judge!
